Public Complaint

Instructions for use of the Public Complaint Submission Portal:
  • Enter the information requested in each section of the complaint form.
  • Anonymous complaints are accepted and will be investigated if sufficient identification information for the licensee is submitted. We must be able to properly identify the nurse in question before we can proceed.

Please note, copies of the complaint will not be available once they have been filed pursuant to G.S. 90-171.37C as they are a part of the investigative file, confidential, and not subject to discovery. Should you wish to have a copy of the complaint for your records, please save the form prior to submission.
